Do you acknowledge that you are surrendering this pet and that we will rehome to a new family and home. You will not in future request the return of this pet or financial payment from CPR for this pet, and you agree to sign a NSW Government change of ownership form to transfer this pet to CPR.

I certify that I am the owner of the above-mentioned surrendered animal, and that today I transfer that ownership to Cowra Pet Rescue. As such I understand that I relinquish all rights to this animal.
